Vabysmo (faricimab) Intravitreal Injection: What Every Patient Should Know

"Vabysmo" and "intravitreal injection" aren't words most people hear every day, so if your eye doctor just used them, a bit of confusion is completely normal. Add in the idea of a needle near your eye, and it's no surprise most patients walk away with the same two thoughts in their mind: what exactly is this treatment, and will it genuinely improve my vision?
What Is Vabysmo?
Vabysmo is the brand name for faricimab, a medicine injected directly into the eye to treat retinal conditions that threaten central vision. It belongs to a class of drugs called anti-VEGF therapies, but with an important twist — Vabysmo was the first medicine of its kind approved to block two separate damage pathways at once, instead of just one.
Here's why that matters. In several retinal diseases, the eye starts overproducing two proteins: VEGF-A and angiopoietin-2 (Ang-2). Both encourage abnormal, fragile blood vessels to grow and leak fluid into the macula — the small central part of the retina responsible for sharp, detailed vision. That leaked fluid is what causes the blurring, distortion, and gradual vision loss patients notice.
Vabysmo is a lab-made antibody engineered to attach to and neutralize both VEGF-A and Ang-2 at the same time. By calming both signals rather than just one, it's designed to stabilize blood vessels more thoroughly, reduce swelling, and protect the vision that's still there.

Patients are often surprised by how common, and how connected, these root causes are.
Wet AMD tends to develop with age, and risk climbs further with smoking, a family history of AMD, high blood pressure, and certain genetic factors. It's the "wet," or neovascular, form of AMD — where abnormal vessels break through a layer beneath the retina — that requires injections like Vabysmo.
Diabetic macular edema is a complication of diabetic retinopathy. Years of elevated blood sugar weaken the walls of tiny retinal blood vessels, causing them to bulge, leak, and eventually swell the macula. Anyone with type 1 or type 2 diabetes can develop it, and risk rises the longer blood sugar has run high.
Retinal vein occlusion happens when a vein draining blood from the retina becomes blocked, often linked to high blood pressure, high cholesterol, diabetes, or clotting disorders. The backed-up blood flow forces fluid out of the vessels and into the macula.
Managing the underlying condition — blood pressure, blood sugar, cholesterol — always remains part of the bigger picture. Vabysmo addresses the direct, vision-threatening consequence: the leaking and swelling itself.
In all three, the core problem is the same: leaky, unstable blood vessels flooding the macula with fluid. Vabysmo is delivered as an intravitreal injection, meaning it's placed directly into the vitreous — the clear, gel-like substance filling the middle of the eye — so the medicine can act right where the damage is happening, rather than travelling through the bloodstream first.
What Makes Vabysmo Different from Other Anti-VEGF Injections?
This is usually the first question patients ask, especially if they've heard of older injections like Lucentis (ranibizumab), Eylea (aflibercept), or Avastin (bevacizumab).
Those medications work by blocking only VEGF-A. Vabysmo blocks VEGF-A and Ang-2, targeting two of the biological drivers of vascular instability instead of one. The idea — since backed by large clinical trials — is that calming both pathways leads to more stable blood vessels and a longer-lasting effect, which can translate into fewer injections over time for many patients.
What the clinical trials actually showed
Vabysmo wasn't approved on theory alone. It went through several large Phase 3 trials, each comparing it head-to-head with aflibercept (Eylea):
TENAYA and LUCERNE (over 1,300 patients with wet AMD combined) tested Vabysmo on a flexible schedule of up to every 16 weeks, based on each patient's individual disease activity, against aflibercept dosed every 8 weeks. Vision gains came out statistically equivalent between the two groups, and roughly 4 out of 5 Vabysmo patients were able to stretch their injections to every 12 weeks or longer within the first year.
YOSEMITE and RHINE (nearly 1,900 patients with DME combined) tested a similar personalized dosing approach for diabetic macular edema, again finding vision improvements on par with aflibercept, along with meaningful drying of retinal fluid on OCT scans.
BALATON and COMINO, the trials behind Vabysmo's approval for retinal vein occlusion, showed average vision gains of roughly 17 to 19 letters on a standard eye chart, along with a substantial reduction in retinal swelling.
Longer follow-up studies (AVONELLE-X and RHONE-X) have since tracked patients for up to four years, showing that vision gains and the reduced treatment burden generally hold up over the long term, with a consistent safety profile.
In short, Vabysmo doesn't just match older injections on effectiveness — its dual mechanism was specifically designed, and has been clinically shown, to let many patients go longer between visits without giving up results.
How Soon Does Vabysmo Work?
Onset: Vabysmo doesn't take long to start showing an effect. In clinical trials, patients typically noticed measurable vision improvement within the first month after their initial injection, with fluid on retinal scans beginning to reduce even sooner. That said, healing is gradual — most people see their vision continue to stabilize or improve over the first few months of treatment, not just after a single dose.
What Actually Happens During the Injection?

Numbing eye drops (or sometimes a gel) are applied, so the surface of the eye is fully anesthetized before anything touches it.
Antibiotic eyedrop once or twice prior to the injection.
