Cotton Wool Spots on the Retina: Causes, Diagnosis and What They Mean

If a routine eye exam or a retinal photograph turned up the term "cotton wool spot," it's natural to wonder what that actually means, and whether it's something to worry about. Here's the short version: a cotton wool spot isn't a disease itself. It's a sign, a small, fluffy white patch on the retina that shows up when a tiny area of retinal tissue has been briefly starved of blood flow. On its own, it rarely affects vision. What matters far more is what's causing it, because cotton wool spots are often the retina's way of flagging a health condition elsewhere in the body before it's been diagnosed.

What Are Cotton Wool Spots?

Cotton wool spots (sometimes called "soft exudates," an older and slightly misleading term) are pale white to greyish-white lesions with soft, feathery, ill-defined edges, giving them the fluffy appearance that names them. They're typically small, usually less than a quarter of the diameter of the optic disc, and tend to cluster in the posterior pole of the eye, following the arc-shaped path of the nerve fiber bundles as they sweep toward the optic disc. Because of this, they're frequently found in a peripapillary distribution, meaning clustered around the optic disc itself.



What Layer of the Retina Are Cotton Wool Spots In?


This is one of the most searched questions about cotton wool spots, and it has a precise answer: they form in the retinal nerve fiber layer (RNFL), the innermost, most superficial layer of the retina, made up of the axons of retinal ganglion cells traveling toward the optic nerve. This location is exactly why cotton wool spots look and behave the way they do, and it's also the key detail that separates them from other white or yellow retinal findings that sit in deeper layers (more on that below).





What Causes Cotton Wool Spots on the Retina?


The traditional and most widely accepted explanation is straightforward: a small precapillary arteriole supplying that patch of nerve fiber layer becomes obstructed, cutting off blood flow to a tiny, localized area. This focal ischemia interrupts axoplasmic flow, the constant transport of cellular material along the ganglion cell axons, in both directions. With nowhere to go, mitochondria and other intracellular organelles pile up inside the swollen axons. Under the microscope, this buildup forms a distinctive structure called a cytoid body, considered the histologic signature of a cotton wool spot. Clinically, this localized swelling and organelle buildup is what appears through the ophthalmoscope as a soft, white, cloud-like patch.


Experimental research using microsphere embolization to deliberately block small retinal arterioles found that nerve fiber swelling begins within about an hour of the blockage, and that each lesion develops in distinct zones, a small area of true tissue death in the center, flanked on either side by swollen but still-viable nerve fibers.


Ischemia isn't the only route to a cotton wool spot, though. Axoplasmic flow can also be disrupted mechanically, without any blood vessel blockage at all. Traction from an epiretinal membrane, or direct trauma that lacerates the nerve fiber layer, can physically distort or sever the axons enough to interrupt transport and produce an identical-looking lesion. This distinction matters clinically: cotton wool spots caused by membrane traction typically resolve much faster, often within about a week of the membrane being surgically removed, compared to the eight to ten weeks typically seen with ischemic causes like diabetes or hypertension.


It's also worth noting that the "ischemia causes infarction" model, while dominant for decades, isn't entirely settled. Some researchers have proposed that cotton wool spots may function more as boundary markers, or "sentinels," at the edge of a zone of retinal injury, rather than being the infarcted tissue itself. The practical takeaway for a patient is the same either way, but it reflects that this is still an area of active research rather than fully closed clinical science.





Conditions Associated With Cotton Wool Spots


A cotton wool spot is not considered a normal finding in a healthy eye. In one notable series of patients who had cotton wool spots with no previously known medical history, roughly half were found to have elevated diastolic blood pressure, and about one in five had elevated blood sugar, discovered specifically because of the retinal finding. This is why even a single, isolated cotton wool spot generally prompts a basic health check, not just an eye check.


Ischemic and vascular causes (the most common category):

  • Diabetes — cotton wool spots are one of the hallmark signs of nonproliferative diabetic retinopathy, often appearing alongside microaneurysms, another early diabetic change

  • Hypertension — including severe or malignant hypertension, where larger and more numerous cotton wool spots can signal a hypertensive emergency

  • Retinal vein occlusion (RVO) — cotton wool spots are more prominent in the more severe, ischemic form of central or branch RVO, where they typically appear alongside extensive retinal hemorrhage, and their extent helps indicate how ischemic, and how high-risk, the occlusion is

  • Retinal artery occlusion — since the underlying mechanism is arteriolar blockage, cotton wool spots can appear around branch or central retinal artery occlusions

  • Ocular ischemic syndrome, hyperviscosity states, hypercoagulable states, radiation retinopathy, and acute blood loss


Blood and bleeding disorders:

  • Anemia — retinal hypoxia from low hemoglobin can directly cause nerve fiber layer infarction; the resulting anemic retinopathy can closely mimic diabetic or hypertensive retinopathy

  • Thrombocytopenia and other clotting disorders

  • Leukemia and lymphoma — rare, but cotton wool spots have occasionally been the first presenting sign of a blood cancer


Infectious and immune-mediated causes:

  • HIV retinopathy — isolated cotton wool spots are one of the most common ocular findings in HIV, reflecting a retinal microvasculopathy tied to immune dysfunction, distinct from an active infection (see the CMV distinction below)

  • Autoimmune and inflammatory conditions, including lupus, dermatomyositis, scleroderma, and giant cell arteritis

  • Cat-scratch disease, Rocky Mountain spotted fever, and other systemic infections


Mechanical and other causes:

  • Epiretinal membrane traction and direct ocular trauma

  • Purtscher retinopathy, following severe compressive chest or head trauma

  • Interferon-associated retinopathy in patients on interferon therapy

  • Rare case reports following intravitreal injections

  • Idiopathic cases, which are uncommon and only diagnosed once every other cause has been ruled out



Cotton Wool Spots and CMV Retinitis: An Important Distinction


Because both are associated with HIV, isolated cotton wool spots are sometimes confused with cytomegalovirus (CMV) retinitis, but they're quite different in both cause and appearance. A cotton wool spot from HIV-related microvasculopathy is a small, self-limited, non-infectious finding that typically resolves on its own. CMV retinitis, by contrast, is an active opportunistic viral infection that produces much larger, spreading, granular areas of retinal whitening, usually with accompanying hemorrhage, and it requires prompt antiviral treatment to prevent permanent vision loss. Telling the two apart accurately is one of the more important distinctions a retina specialist makes when examining an immunocompromised patient.




Are Cotton Wool Spots Dangerous? Do They Cause Symptoms?


For the most part, cotton wool spots themselves are visually silent. Most people have no idea they have one until it's picked up incidentally during a dilated eye exam or retinal photography, which is exactly why routine eye exams matter for anyone with diabetes, high blood pressure, or another vascular risk factor. The main exception is when a cotton wool spot happens to sit directly over or very near the fovea, the tiny central point responsible for sharp vision, in which case it can cause a noticeable blur or blind spot.


The real significance of a cotton wool spot isn't the lesion itself, it's what it's telling you about blood flow elsewhere in that eye, and often about a systemic condition that hasn't been diagnosed yet.





Cotton Wool Spots vs. Similar Retinal Findings


Several retinal findings can appear superficially similar to cotton wool spots, especially to an untrained eye. However, they differ in their appearance, retinal location, and underlying cause.

superficially similar to a cotton wool spot, especially to an untrained eye, but they differ in appearance, retinal layer, and underlying cause. The cotton wool spot vs. hard exudate distinction is one of the most clinically useful to know, since the two point toward different disease timelines: cotton wool spots reflect an acute, resolving ischemic event, while hard exudates reflect ongoing, chronic vascular leakage.


It's also worth being clear that a cotton wool spot, by definition, does not itself contain blood, which is what separates it from a Roth spot. That said, cotton wool spots very often appear on the same retina alongside separate areas of hemorrhage, particularly in diabetic retinopathy and ischemic retinal vein occlusion, where both findings result from the same underlying vascular damage even though they're two distinct lesions.





What Do Cotton Wool Spots Look Like on OCT?


On optical coherence tomography, an acute cotton wool spot appears as a focal or segmental area of thickening with increased reflectivity, confined largely to the inner retina and nerve fiber layer, while the deeper retinal layers are relatively spared. This inner-layer-predominant pattern lines up precisely with the idea that the debris causing the lesion is trapped within the ganglion cell axons themselves. As the lesion resolves over the following weeks, OCT can show the affected area thinning out or, in some cases, developing small cystic changes, reflecting the loss of nerve fiber tissue that a cotton wool spot can sometimes leave behind even after it's no longer visible on exam.

Diagnosing Cotton Wool Spots


Diagnosis starts with a complete, dilated fundus examination, where the lesion's classic fluffy white appearance and nerve-fiber-layer location are usually enough to identify it. From there, the real diagnostic work is figuring out why it's there:


  • Fluorescein angiography can confirm the finding by showing an area of capillary non-perfusion directly adjacent to the spot

  • OCT imaging documents the structural changes described above and is useful for tracking resolution over time

  • Widefield retinal imaging (including ultra-widefield systems) can help capture associated peripheral findings, such as areas of non-perfusion in ischemic retinal vein occlusion, that standard fundus photography might miss

  • Systemic workup, guided by the clinical picture, often includes blood pressure measurement, fasting blood glucose or HbA1c, a complete blood count, and, where relevant, inflammatory markers or HIV testing



ICD-10 Code for Cotton Wool Spots


In ICD-10-CM, cotton wool spots are classified under H35.81 (Retinal edema), which specifically includes cotton wool spots as an inclusion term. This code carries no laterality character, so the affected eye needs to be documented separately in the clinical note. Because a cotton wool spot is a sign rather than a standalone diagnosis, coding guidance generally recommends also coding the underlying cause where one has been identified, for example, the relevant diabetic retinopathy or hypertensive retinopathy code, alongside H35.81.





How Are Cotton Wool Spots Treated?


There is no direct treatment for a cotton wool spot itself, no laser, injection, or medication targets the lesion directly. Instead, management is entirely about identifying and treating the underlying cause: tightening blood sugar control in diabetes, managing blood pressure in hypertension, working up and treating an underlying blood disorder, or, in mechanical cases, surgically addressing the epiretinal membrane that's causing traction.


Left alone, most cotton wool spots resolve on their own, typically within four to twelve weeks. The exact timeline depends on the cause: lesions related to membrane traction tend to clear the fastest, often within about a week of the membrane being removed, while those related to diabetes or hypertension more typically take eight to ten weeks to fully resolve. Interestingly, some research suggests the mere presence of cotton wool spots doesn't necessarily predict how quickly diabetic retinopathy itself will progress, but it does confirm that blood flow to that part of the retina has been compromised, which is exactly why ongoing monitoring matters even after the visible lesion is gone.
