What is Vitrectomy Surgery?
Vitrectomy, medically called pars plana vitrectomy (PPV), is the most commonly performed retinal surgery. It treats diseases affecting the retina and the vitreous — the clear gel that fills the inside of the eye.
The procedure involves creating three tiny self-sealing openings in the white part of the eye using specialized micro-incision ports called trocars. These give the surgeon access for delicate microsurgical instruments while preserving the structural integrity of the eye.

Conditions Treated with Vitreoretinal Surgery
Diabetic Vitrectomy
Advanced diabetic retinopathy can cause vitreous hemorrhage, where blood fills the eye and severely reduces vision, or tractional retinal detachment, where scar tissue pulls the retina away from its normal position.
Diabetic vitrectomy removes the blood and scar tissue, relieves the traction on the retina, applies laser treatment where required, and helps preserve or restore vision while reducing the risk of further complications. Retinal Detachment Surgery
A rhegmatogenous retinal detachment occurs when a retinal tear allows fluid to pass beneath the retina, causing it to separate from the back of the eye. This is an ophthalmic emergency that needs urgent evaluation and treatment.
Depending on the nature of the detachment, retinal detachment surgery may involve vitrectomy, laser photocoagulation, cryotherapy, intraocular gas, silicone oil, or a combination of these techniques to reattach the retina and preserve vision.
Macular Hole Surgery
A macular hole is a small full-thickness defect at the center of the retina that causes blurred and distorted central vision.
Modern macular hole surgery involves vitrectomy with meticulous internal limiting membrane peeling, followed by placement of a temporary gas bubble. With contemporary techniques, anatomical closure rates exceed 90% in appropriately selected patients, and many experience significant improvement in central vision.
Epiretinal Membrane (ERM) Surgery
An epiretinal membrane, also called a macular pucker, is a thin fibrous membrane that develops over the retinal surface and causes blurred vision, image distortion, and difficulty reading.
Vitrectomy with microsurgical membrane peeling relieves the retinal distortion, allowing gradual improvement in vision over the following months.
